MIDI update files cause more problems than they solve, please don't use them.
(they were designed for when Windows 98 was the latest version, MIDI behavior in Windows was different then).

    TM-D4000
    Main V1.15
    Panel V1.13

Release Notes for Tascam Digital Mixer TM-D4000
    [V1.13]

    Problems fixed:

    1) Set stereo fader at desired position (for instance 0 dB).
Turn the power off. Set the stereo fader to the bottom.
Turn the power on. Signal is outputted from the stereo out
by 0 dB even the fader is minimum.
If stereo fader is moved slightly, it starts to work nomally.

    2) The TM-D4000 cannot detect the DA-78HR by AUTO DETECT function.

Release Notes for Tascam Digital Mixer TM-D4000
    [V1.12]

    Problems fixed:

    1) If automation mode of stereo master is changed by
the host computor, some lower dots of C.R. monitor
meter on the TM-D4000 will light falsely.
This problem does not occur when using the TM-D4000 stand alone.
This problem does not affect the audio signal.

    2) Jitter componemts of MTC output converted
    from SMPTE has been improved.

    3) When the snap shot is recalled, change of audio data of
STEREO OUT is delayed than other channels.

    4) Wrong spelling in the LCD view. "Dencity" has been corrected to "Density" in EFFECT LIB/Reverb.

    (no public V1.14)

    Problems corrected by V1.15
    1. If a number of Fader Group’s are programmed over
the fader layer, move of the slave fader on the
LCD stops or becomes slow. No problem on the actual audio level.
    2. After the Stereo Link is released, audio level
and PAN indication should go back to the center.
However, audio of even channel is assigned fully
right while the indication of PAN in the LCD is
    center.
    3. TM-D4000 cannot operate Punch Out with
the DTRS machine through the RS-422 port.

Contact TASCAM PARTS to get a quote for a set of ROMs.

    From the technote:

Refer to "10.8 Updating the system software" in the owner's manual.
    Notes:
    1) MIDI files should be reproduced in the following order.
    1. Flash
    2. Panel
    3. Main
    4. Remote

    2) Only the firmware V1.03 cannot be upgraded by using SMF. If your TM-D4000 is V1.03,
you should re-program the ROM's by the ROM burner or replace the ROM's.

Main V1.03 is the problem I think.

Which TASCAM support are you contacting? Every email through the tascam.com (America) website should generate an auto response email with a reference number, we can use that to find whether we replied or not.
For tascam.eu, I'd need to ping our guys over there.
